Self-assessment tool

The Parkinson’s Self-Assessment Tool is designed for people with Parkinson’s to complete over a 2-week period and then share with their nurse, doctor or other healthcare professional.

The idea is that you pick some core daily activity such as walking up and down stairs, or speaking clearly. You then record how you managed that on their simple tick list over a two week period. There is also space to record some more general points such as how well you slept and how effective you felt your medication was. It all looks quite clear and seems straightforward to complete , though doing it consistently for 2 weeks may feel like a bit of a stretch. Hopefully it will enable you to maximize the benefits from your medical appointments.
